• Comb or cut at least 15 strands of clean hair.  Please be sure hair does not have styling products as they may be show up in the scan.  Place hair into a clean plastic bag and seal the bag.
  
  
• Use a clean cotton swab to rub the inside of the cheek for approximately one minute.  Place the swab in the plastic bag with the hair and seal the bag. 
  
  
• Complete the client information form. 
  
  
• Read and sign the release form.
  
  
• Place the above items in an envelope and mail it to the above address.

Once I have received the sample and have completed the BioScan analysis and food sensitivity testing, I will call you to schedule an appointment to review the results and recommendations. Together we will determine a course of action and what homeopathy and supplements you would like. 

During your appointment I will obtain payment, and following the appointment I will mail the report, the foods list and desired products.
